• Any person making default in complying with the provisions of this section shall be punishable with fine which may extend to Ten Lakh Rupees.

• I agree that the cover shall be voidable at the option of the Aditya Birla Health Insurance Company limited, in the event of any untrue or incorrect statement, misrepresentation, non-description or non-disclosure in any material particular in the enrolment form/personal statement, declaration and connected documents or any material information has been withheld by me or anyone acting on our behalf to obtain any benefit under this cover.
• I further consent and authorize Aditya Birla Health Insurance Company limited and/or any of their authorized representatives to seek medical information from any hospital / Medical Practitioner / Insurer / any of the related entity that I have attended or may attend in future concerning and disease / illness / injury.
• In case of any claim made under the policy, no premium shall be refunded on cancellation of the policy.
• I consent to provide a valid age proof and identity proof at the time of claims or any other time when required by the Aditya Birla Health Insurance Company limited
• I consent to receive information from the Aditya Birla Health Insurance Company limited through physical, electronic or telecommunication means from time to time.
• I have not been diagnosed with or had any signs/symptoms or advised/taken treatment or surgery for any of the following: Heart Disease, Peripheral Vascular Disease, procedures like Angioplasty/PTCA/By Pass Surgery, Diabetes, High blood pressure, Anaemia/Blood disorder, Tuberculosis (TB), any Respiratory / Lung disease, Disease of Eye, Ear, Nose, Throat, Thyroid, Paralysis, Polio, Cancer, Tumour, Lump, Cyst, Ulcer, Disease of Kidney, Liver/Gall Bladder, Pancreas, Digestive tract, Breast, Reproductive/Urinary system or Complications of pregnancy, Disease of the Brain/Spine/ Nervous system, Epilepsy, Joints/Arthritis, Congenital/Birth defect, Physical deformity/disability, HIV/AIDS, Sexually Transmitted Disease, Any other pre-existing disease/abnormal test reports (apart from viral fever or common cold, malaria or common diarrhoea)
• I hereby declare that I have understood the contents on this webpage including the entire text, features, waiting periods for coverage and Pre-Existing Diseases along with disclosures, termsconditions, respective restrictions on Sum Assured for select disease treatments, etc and all other documents incidental to availing the health insurance from Aditya Birla Health Insurance Company.

If the Insured Person is still not satisfied with Our redressal, he/she may approach the nearest Insurance Ombudsman.
